By submitting this document I understand that participation in the  School Sports Program is contingent on my grades and behavior. My teacher in each subject area will mark the eligibility form weekly. I further understand that I must maintain a "C" or better average in all subjects and conduct.

I also understand that I must attend practice and water my sports uniform at each game or I will not be eligible to play.
